Some of the most common causes of infertility are problems with ovulation, anovulation (cannot produce mature eggs), the age of the woman, sexually transmitted diseases, endometriosis, polycystic ovary, hormonal problems, obstruction of Fallopian tube, sperm rejection or unknown causes.

The most common causes are seminal infection that blocks the exit of sperm, sexually transmitted diseases, hormonal alteration, or personal habits that produce low sperm count or low-quality sperm. There are more severe cases such as azoospermia which means that there are no sperm or unknown causes in the semen.

Other causes can be due to infections, excessive exercise, extreme obesity or anorexia, the abuse of alcohol, tobacco or other drugs can cause a decrease in fertility. Also, stress in workplaces with an inadequate environment where employees are in contact with pesticides or other toxic substances.

It is recommended to freeze the ovules when women want to delay motherhood; freezing the ovules is an option to maintain good quality ovules until the moment you want to be a mother. Then, an in vitro insemination can be performed. This option is also recommended for patients who are going to have cancer treatment because these treatments can cause infertility.

Both are assisted reproduction techniques, but they are not the same. In artificial insemination, ovulation is monitored so that when the egg is released and travels through the fallopian tubes to the uterus, the sample of semen that was obtained from the man is placed in the uterus of the woman through a catheter. The procedure is simple and non-invasive. On the other hand, with in vitro fertilization, it is necessary to stimulate the ovaries in your body for two to three weeks to generate more and higher-quality ovules in order to increase the chances of conception. During this time your doctor will monitor the progress of the therapy to confirm you are receiving the right stimulation or to adjust as necessary. Once your body is ready, your doctor will give you a mild sedation to extract your ovules, ideally 10 to 20 ovules, through a minimally invasive and ambulatory procedure, this means that no hospitalization is required. In parallel, the semen sample is acquired and analyzed to select only the highest quality sperm based on mobility and shape and seminal secretions are eliminated. Fertilization is done in vitro in a certified laboratory, not in your uterus as with artificial insemination, and then your doctor will introduce the pre-embryo 2 to 5 days after fertilization in your uterus to continue its development (similar to artificial insemination).

A gynecologist can help in sharing your medical history with your fertility doctor. However, the extraction of ovules, in vitro fertilization, and placement of the pre-embryo in your uterus must be done by a specialist in this field to maximize your chances of success.

There are two types of techniques:

• Traditional in vitro fertilization (IVF): after obtaining the ovules and several sperms (50,000 to 100,000), they are placed in vitro in the lab and you wait for the sperm to fertilize the ovule before placing the pre-embryo back in the uterus.
• Fertilization in vitro with micro intracytoplasmic sperm injection (ICSI): only one sperm is needed, which is injected inside the egg to achieve fertilization. This technique has higher success rates than traditional IVF because fertilization is facilitated by your doctor.

This may be the case if two pre-embryos are placed in the uterus to increase the chances of a successful pregnancy, although It is preferable to make several attempts with only one pre-embryo than placing several pre-embryos in the uterus at once. However, there is a higher probability of having a multiple pregnancy in artificial insemination after an ovarian stimulation treatment since there is no control over how many ovules will be fertilized once the sperm sample is placed inside the uterus.

Typically, some ovules or pre-embryos are frozen during the first cycle in case there is a need for more attempts. This makes it easier for women because there is no need to re-extract eggs and your doctor only needs to unfreeze the ovules and sperm to proceed with the in vitro fertilization and placement of the pre-embryo in your uterus again. Likewise, if your first attempt was successful and you wish to wait a few years to conceive another child, the lab can keep your frozen ovules or pre-embryos for future attempts.

During your first visit, your doctor will conduct a comprehensive evaluation once again to confirm your IVF treatment and you will be given your medications with clear instructions so that you begin taking them in the 2nd day of your period. To make sure your medication is working, or to make any dosing changes, your doctor will need to measure your egg follicles with a vaginal ultrasound 5 days after you begin treatment as well as on day 8 and 11. Taking these steps will maximize your chances of a successful pregnancy.

The first step is your case review to determine the cause of infertility. Once the problem has been identified, your doctor will design a medication protocol to stimulate your to generate more ovules with better quality during that period to ensure they are ready for conception. Your fertility doctor will ask you to get a few ultrasounds to determine the best time to perform the extraction of ovules.

The extraction of your ovules is a process that does not require hospitalization and just a mild sedation to relieve any discomfort, you may return to your hotel after the procedure to rest. Later, on the same day, the in vitro fertilization is performed with the highest-quality semen from the sample obtained. Once the ovules and sperm sample have been collected and selected, there are two in vitro fertilization options:

• The first option is to place the eggs and sperm in a Petri dish to let the sperm fecundate the ovules, and the next day you will know if any ovule was fertilized.
• The second option, typically for more severe cases, is to use an intracytoplasmic micro sperm injection (ICSI) which consists of injecting only one sperm into the ovule, which increases the chances of conception because your doctor makes sure the egg is successfully fertilized.

As sometimes more than one cycle of IVF is required before a successful pregnancy, you can freeze your eggs from the first extraction to avoid extracting more eggs for a second or third attempt. After the ovule has been fertilized, the pre-embryo is placed in the woman’s uterus to continue its development. The process of implanting the pre-embryo is not painful and does require sedation, you will be in a gynecological position and your doctor will place the fertilized egg inside your uterus.
